You can complete your form online from any computer, tablet, or smartphone with an internet connection. If you do not wish to submit the form online, you can request a paper version from the school.

This form must be completed each school year for all children attending pre-k through high school — it is VERY important for all families to submit, regardless of income. It’s quick and easy, and you only need to submit once for all children in your household attending school.
